Horizon Community Service was created for thier community, friends and families who need a better way to access individualized disability support. It provides a wide range of tailored, flexible, and quality disability services and support coordination for people living with disabilities of their choice and needs.

Horizon SDA help Participants to get into an SDA home, where they can live with dignity and respect, with as much independence as possible.

We provide NDIS creative programs for all NDIS participants who are self-funded, or plan managed. Here are some of the programs we offer. 1. Dream catcher program. 2. Macrame program 3. Clay program 4. Painting program These interactive programs run for 2 hours, and we will provide morning tea, we are based in Dayboro.
