ccm. is a NDIS-registered provider based in Perth. We provide Specialist Support Coordination, Occupational Therapy reports, Psychosocial Capacity Assessments, and Acquired Brain Injury training. Our experienced team of health and social care professionals specialise in Acquired Brain Injury and Psychosocial disability; however we work with all disabilities and age groups.

    
We provide Support and Specialist Support Coordination to NDIS participants. Our team of practitioners are here to help participants understand and implement their NDIS plan, coordinate multiple services, resolve points of crisis, and overcome barriers to service delivery.

Our Occupational Therapists specialise in NDIS Functional Capacity Assessments (FCA) and Housing Reports. We provide these services in the Perth and Mandurah areas.

For people with Psychosocial disabilities, a standard FCA may not capture the entire picture. We offer Psychosocial Functional Capacity Assessments (PFCA) for people with Psychosocial disability in the Perth metro area.

Our training workshops aim to develop the knowledge of Acquired Brain Injury (ABI) for those working in the disability sector, their families, and carers. We demonstrate effective techniques that enable trainees to better engage with their client or family member, develop their skills, and understand behaviours.
